There will be an additional $15 fee for any forms or letters provided for patients.

No Shows

Patients will be charged a $65 no-show fee for any no-show without a prior 24-hour cancellation for any service scheduled. The no-show fee must be collected prior to any new appointment needing to be scheduled.

POLICY FOR MEDICATION REFILLS

If a patient needs a medication refill for non-controlled medications, but has missed their previous appointment, a one-time courtesy medication refill can be made within a 6-month period. In order for the refill to be sent, the patient is required to schedule a follow-up visit. This patient will be responsible for a $45 charge at this time.

​

All patients needing a refill for controlled medications (benzodiazepines, sleep medications, and stimulants) are required to have a scheduled appointment.
